Hike the Reserve: Photograph and Affiliation Release Form (Under 18)

I am the parent or legal guardian of the minor (“Subject Minor”) named below and I hereby grant Save the Redwoods League, a California non-profit corporation (“League”), the absolute right and permission to take, use and publish any photograph(s), film image(s) and any other audio or video production(s) in any format, using any media and from any electronic device, hereafter referred to inclusively as “image(s)”, of the Subject Minor for whom I am the parent or legal guardian of, created for internal and/or external purposes, including but not limited to, promotional materials, publicity, solicitation efforts and online use.

I understand that the image(s) may be used in a publication, print ad, direct-email piece, electronic media (i.e. video, CD-ROM, podcast and/or online) or any other form of internal or external publication or promotion. I hereby acknowledge the League’s right to have editorial control and its right to edit, alter, crop or treat the image(s) in any manner at the League’s discretion. I waive any right to inspect or approve the image(s). I acknowledge that the League may choose to not use the image(s) at this time but may do so at the League's discretion on any other date. I agree that neither I nor the Subject Minor are entitled to compensation or acknowledgment of any type in connection with any League use of the images(s). I further understand and agree that, in connection with the image(s), the League (initial one of the following):

I and the Subject Minor hereby release the League, its directors, officers, employees, volunteers, agents, designees and the photographer, videographer and recorder from, and agree not to sue the foregoing, for any and all claims, liability, injury or loss in connection with such use, including but not limited to, any claims for defamation, invasion of privacy, violation of right of publicity or any other violation of my and the Subject Minor’s personal or proprietary right.

I am over the age of eighteen (18), I understand the content of this Release and I agree that this Release shall be binding upon myself, the Subject Minor and the heirs, legal representatives and assignees of myself and the Subject Minor.
